Muscle IQ Registered Massage Therapy Inc. ("Muscle IQ", "we") respects your privacy. As a registered massage therapy clinic in Ontario, we protect your personal health information under the Personal Health Information Protection Act, 2004 (PHIPA) and the standards of the College of Massage Therapists of Ontario.
What we collect
To give you safe, effective treatment, we collect only what we need:
- Your name, contact details and date of birth
- Your health history, the reason for your visit and your treatment notes
- Insurance and billing details, when you ask us to bill your insurer
How we use it
- To assess you and plan and give your treatment
- To book appointments and contact you about them
- To bill you or your insurer and issue receipts
- To meet our legal and professional duties
We never sell your information or use it for advertising.
Who we share it with
We share your information only with your consent, or when the law requires or allows it. For example, with your insurer when you ask us to bill them, or with the College of Massage Therapists of Ontario if it requests records.
Booking and records
Online booking, gift cards, health history forms and clinical records are handled by Jane App, a Canadian practice management system built for health clinics. When you book or fill in a form, you are using Jane, and Jane's privacy policy also applies.
How long we keep it
Records are kept securely and retained for at least ten (10) years after your last visit, as required by the College of Massage Therapists of Ontario. After that, they are destroyed securely.
This website
This website does not use cookies, analytics or tracking, and it has no forms. Our fonts load from Google Fonts, which may receive your device's IP address when the page loads.
Your rights
You can ask to see or get a copy of your records, ask us to correct them, or withdraw your consent, subject to legal limits. Ask us by email or phone.
